A/C was lead in flight of 3. As darkness began to fall, the flight entered a light rain. As A/C could clearly define a cloud bank ahead, they descended to 500'. A short time later the ceiling dropped to 200' and limited contact with the ground. A/C turned to the right and toward the river, rolling out of the turn at 70 knots and 150' AGL. A/C hit a large tree and then several other trees, impacted the water in an upright position and began to sink.

A/C apparently was flying SE and descending while approaching a ridgeline that runs north-south, striking tree tops at the summit of an elevated portion of the ridgeline, continued approx 200 meters and impacted with the ground.

On a CA mission to Red Devil Road. While descending, an explosion occurred. Crew decided to head for FSB Sheppard. Crew reported they had overshot Sheppard and were going on to Vandergrift, rather than circle around. A/C proceeded down the Quang Tri River when engine failed, and successfully autogyrated into the river. During the rescue attempt, as CPT Taylor was being pulled off the skid, a strong river undertow sucked him under water.

A/C was assisting a group of Army Rangers under attack, then destroy left behind ammunition and mines near Hill 1015 (Dong Tri Mtn) and were hit with AA fire, causing the A/C to crash somewhere around the hill, about 6 mi SW of Thon Khe Xeng. A/C was hit in the underside by enemy fire during gun run. Escaping fuel caught fire and A/C nosed up, gaining perhaps 150' before descending in a vertical right turn to ground impact. *Remains were found in 2008.

A/C was covering extraction of the last of 4/1 ARVN troops from Laos during Lam Son 719. A/C was leading other aircraft to LZ when NVA fire became intense. A/C shuddered and pilot stated that he lost the engine and transmission. A/C hit the ground with low rotor RPM and the blades then came off pretty much in tact still on the mast along the left side of the A/C, began to burn from the right side and then was engulfed in flames.

A/C was on an afternoon check ride and were landing from the North at Can Tho when the A/A Porter entered traffic requesting permission to land. Porter was on VHF and the Cobra was on UHF. Cobra was given permission to autorotate, the tower immediately saw the two on a mid air collison and tried to have the A/A go around when the Cobra made contact from above killing the two pilots in the Cobra and the 6 occupants in the Porter.

A/C was on an armed recon mission W of Quang Tri. During an attack on a target, A/C was hit by enemy ground fire. Pilot radioed that the engine oil bypass caution light was on, and that he would attempt to return to Khe Sanh. On the return attempt, the A/C began to smoke and burn, and the crew attempted to land. Just prior to touchdown, the A/C exploded and crashed, followed by intense fire and ammunition detonation.

A/C initiated a steep dive toward the friendly APC's. Recovery from the dive was initiated, but the A/C struck the ground. After initial impact, the tailboom, M/R and transmission, and engine separated from the A/C. Cabin compartment then bounced several times and rolled end-over-end, finally coming to rest in a ravine.

A/C appeared to be in a normal autorotative glide, when A/C was observed to assume a radical decelerating attitude with a subsequent loss of airspeed and rotor RPM. A/C's momentum carried it just clear of the trees where it settled vertically with zero airspeed and rotor RPM.

A/C was flying wing on a light fire team providing security around Cu Chi base camp for visiting VIPs. A/C radioed lead and stated they had possible short shaft failure. A/C was attempting to land on active runway when a loud ping was heard followed by drop in altitude. A/C entered autorotation in attempt to land on Hwy 8A or the open rice paddys adjoining road. On short final, at 100-155' altitude, pilot turned toward paddy to avoid vehicle on road. Skids became tangled in barbed wire fence, struck ground on its right side approx 10m from the fence and burst into flames.

The M/R assy was spinning in the air approx 100m to the east of the fuselage. With no detectable forward travel after impact, the A/C came to rest on the left wing store and left side of the broken mast. M/R assy remained airborne 5-10 seconds after Cobra crashed. TO died 3 days later of injuries.

A/C struck ground hard causing M/R blades to flex down into the copilot compartment, xmsn and mast separated and shortly afterward fuel cells ruptured and burst into flames.

A/C crashed on an escort mission. The engine failed and the A/C entered autorotation. While in an autorotative dive turn to the left, the pilot noticed that the RPM needle was directly downward. Pilot pulled pitch to put the RPM in the high green. A/C descended vertically from 15' and contacted the hillside very hard with the left skid, on a westerly heading. A/C then flipped over to the right and came to rest inverted in water with the right side of the A/C against the bank. A/C was 3/4 submerged with the canopy completely submerged.

A/C was part of a hunter-killer team with an OH-6A that left Tay Ninh west heliport at 1500 hours. They proceeded to AO and conducted a visual recon for approx 30 minutes. An OV-10A in the AO leveled off at 1500' when they spotted the AH-1G approaching head-on approx 150m away. OV-10A made abrupt full left movement to avoid collision but AH-1G impacted the right wing and outboard engine, causing AH-1G to explode or disintegrate. OV-10A crew ejected safely.

M/R head and blades were on ground approx 50' to the left and rear point where acft impacted.

At about 1405 hours, while making a pass on an enemy gun position, they were hit by 37mm AA fire from the gun emplacement and the A/C exploded in mid-air. They were about 1500' AGL when the explosion occurred, separating the tailboom and one M/R blade from the A/C. A/C spun to the ground on fire and impacted, and seconds later, the ammunition onboard detonated.
