The flights were in a left hand holding pattern at approx 1,200' MSL awaiting instructions to proceed to a PZ. A/C was seen to execute a flare, yawed to the right, then began a descending left spin, making almost 4-360 degree turns prior to impact.

Soon thereafter the pilot lost control of A/C and observed its entry into a steep dive. A/C recovered from dive with G-forces sufficient to cause at least the CE to black out. Shortly after leveling off from the dive, the A/C struck a steep hillside deep in a river canyon.

The A/C skimmed along the jungle canopy until it collided with an even taller tree, then fell off to the side, passing downhill along the side of a ravine before crashing into a small river, and immediately burst into flames.

Approx 10 minutes out of Dong Tam, White 5 was observed to descend in a relatively level attitude with the M/R turning very slowly. A/C struck the ground in a slightly nose low attitude in a rice paddy with apparently very little forward motion. AC and P were pinned in the wreckage by the instrument panel and passenger compartment was on fire.

Due to inclement weather and poor visibility, the CA mission was aborted. During the return flight, the two acft became separated while attempting to return to the Song Be airstrip. Radio contact was lost with A/C after the last contact at 2025 hours on August 1. At that time, the AC indicated that he was diverting to Binh Hoa airbase. When A/C failed to arrive at either base, search efforts were begun, and continued for 3 days. On August 6, wreckage of the A/C was discovered.

A/C was enroute to rendezvous with the Firefly A/C north of LZ Sally. A/C struck the ground in a near level attitude and at a high rate of descent, causing the tailboom to shear. A/C impacted a rice paddy dike, somersaulted to the left over the nose. The red M/R blade struck the ground and failed causing the transmission to shear. A/C continued somersaulting and turned over on the roof, approx 33' from the center of impact. Fire was nearly instantaneous.

During the autorotation, the pilot watched the RPM to such an extent that he lost sight of the intended LZ. The A/C hit the trees with an excessive rate of forward movement. An intensive search was initiated, but failed to reveal any trace of the A/C or the 5 missing. 2 bodies (Gubbins and Moore) were recovered later in shallow grave.

While on strip alert with blues at FB north of Tay Ninh, a gunship (#66-686) from another unit went IFR in dust and T/R struck M/R blade of #036 knocking off #686's T/R causing it to spin right. Tail boom then struck tailboom of #061. Flight terminated with #686 lying on left side 180 degrees from direction of landing and right of #061.
