At approx 200-300', engine lost power and autorotation was initiated. M/R made contact with trees, causing A/C to hit on the left side and roll over 2-3 times.

A/C was flying wingman in a light fire team and was flying at 200' and 70 KIAS when 2 or 3 loud cracking sounds were heard. Ground witnesses observed T/R blades being thrown, A/C turned 45-deg. to right and assumed excessive nose-low attitude. Pilot attempted to reach a clearing when contact with ground in a tail low attitude, causing T/B to break off approx 4' before final impact in a rice paddy. Post fire crash resulted 4 minutes after impact.

On the fourth test flight, A/C lifted off Chu Lai's Runway 32 heading NW and climbed to 150' and made a right turn to crosswind, followed by a further descending right turn to downwind leg. The A/C did not recover from the descending turn and impacted the water offshore.

A/C was flying chalk 5 in a flight of 5 lift ships with a 2nd flight of five 30-60 seconds to the rear. The flights were in a left hand holding pattern at approx 1,200' MSL awaiting instructions to proceed to a PZ. A/C was seen to execute a flare, yawed to the right, then began a descending left spin, making almost 4-360 degree turns prior to impact.

A/C was performing a combat assault and was enroute to the LZ. #893 received instructions to break formation after troops insertion to pick up 3 medevacs who were in a location 1000m SE of the LZ. A/C was hovering toward medevacs location and approx 100m from them began to slow down, executed an extreme nose high flare causing T/R to hit rice paddy, lost directional control, turned 180 deg. M/R struck paddy causing xmsn, mast and rotor system to go through portions of forward cabin where pilot sat.

A/C departed Camp Evans POL to conduct a mortar patrol mission in the vicinity of LZ Nancy. After take off, at 500' altitude the A/C went IFR in clouds and rain. Soon thereafter the pilot lost control of A/C and observed its entry into a steep dive. A/C recovered from dive with G-forces sufficient to cause at least the CE to black out. Shortly after leveling off from the dive, the A/C struck a steep hillside deep in a river canyon.

A/C struck the ground in a slightly nose low attitude in a rice paddy with apparently very little forward motion. AC and P were pinned in the wreckage by the instrument panel and passenger compartment was on fire.

Due to inclement weather and poor visibility, the CA mission was aborted. During the return flight, the two acft became separated while attempting to return to the Song Be airstrip. Radio contact was lost with A/C after the last contact at 2025 hours on August 1. At that time, the AC indicated that he was diverting to Binh Hoa airbase. When A/C failed to arrive at either base, search efforts were begun, and continued for 3 days. On August 6, wreckage of the A/C was discovered.

A/C departed on its first combat mission at 0630, logging approx 3 hrs flying time. Upon return from the last mission, approx 1050 hours, the A/C went into the POL at Camp Evans. After refueling, the AC took off and experienced a power loss after reaching 50 knots. AC closed throttle and sat up an autorotation, touched down between the perimeter wire with considerable forward speed. The wire stretched and then flipped the helicopter onto its left side causing major damage. CE sustained fatal head injuries during the crash.

Level off was made at approx 1100' indicated. During some portion of the base leg, altitude of 900' was noted. During final approach A/C passed through several layers of smoke or clouds, within a few seconds A/C hit the water severing a portion of the T/B.

A/C continued somersaulting and turned over on the roof, approx 33' from the center of impact. Fire was nearly instantaneous.

During the autorotation, the pilot watched the RPM to such an extent that he lost sight of the intended LZ. The A/C hit the trees with an excessive rate of forward movement. An intensive search was initiated, but failed to reveal any trace of the A/C or the 5 missing. 2 bodies (Gubbins and Moore) were recovered later in shallow grave.

As A/C touched down at the LZ, NVA troops stood up all around and began firing at the A/C as ARVN troops were off-loading. After all ARVN deployment and at approx 8-10' off the ground, the A/C was hit by either a 57mm recoilless rifle or a direct hit mortar fire, burst into flames, and crashed. *Hill's remains identified by DPAA on 14NOV15.
